1 Define the problem and the gap
Say where it is now, where it needs to be, and by when.
2 Break it down with data
Break the number down with counts: by time, job, place and likely cause.
3 Set a target against the opportunities
Using the data, say how much of the gap you can take out.
4 Prioritized problem and point of occurrence: why it happens
Pick one piece within your authority, say where it happens, and ask why five times.
5 Countermeasures, ranked
List several countermeasures and rank them. Does each one remove a cause or only mask it?
6 Plan: put them in place
Say who does what, by when, and when you expect results. Trace each action to a cause.
7 Check the process and the results
On the check dates, ask: are people doing it, and is the number moving?
8 Standardize and share
Lock it in for the next new hire, tell who else should know, and look for the next place it happens.

What the hosted app adds
